NEW PATIENTS: READY TO BOOK?
1. You need to pay a new patient deposit prior to booking your appointment.
Please pay the $100 deposit when scheduling your appointment request. Your appointment will be confirmed after receiving your deposit. Requests without a deposit will be removed as they prevent other patients from booking that time.
A. Confirm you are in WA state
B. If you are outside of washing state you will need to book for consultation services via another link. Stay tuned.
2. Your appointment will be confirmed once we verify you've paid the $100.00 deposit fee.
Pay by clicking on "home" on the top left of this screen and scrolling down to Pay Now. Put FOC deposit in the memo. The fee can go to the cost of the visit or refunded if billing insurance. It will be forfeited if:
A. Forms are not filled out on time.
B. You reschedule less than 48-business-hours M-F before your visit
C. You do not show for your visit.
**If adding yourself to the wait list be sure to add your phone number so we can call you.
3. Once your appointment is confirmed you will get an email to join Charm.
A. Create a new Charm account by clinking on the email link and answering the questions.
B. If you already have a Charm account you still must click on the email link, then login with your username and password to link my business to your existing account.
4. Once you are in the Charm Portal you can now fill out the Questionnaires and Intake Forms. Please fill them out completely at least 24-hours before your appointment.
