Patient Forms
All patient forms / questionnaire's are easily located inside the Charm patient portal.
Login to fill out all the forms at least 24-hours before your appointment. If forms are not filled out 24-hours before your appointment you will be rescheduled.
If you are having trouble please reach out to the office.
Note: There are close to 30 pages of forms. We prefer that you fill them out on the Charm portal. We prefer not to receive paper copies. If you require that we mail them to you, please call the office to make a $5.00 payment to cover the associated costs.
Insurance
Our providers are contracted with the following insurance companies:
- Premera Blue Cross
- Lifewise of Washington
- Regence Blue Shield
- Other Blue Cross and Blue Shield insurance companies
- First Choice
Contact your insurance provider to see if Naturopathic visits and Acupuncture visits are covered by your plan.
It is the patient's responsibility to verify that their insurance covers the services provided by our doctors.
Out of State Insurance:
Out of state patients may bill insurance when the visit is in person. Out-of-state patient may only do telemedicine if they declare they are in Washington state. We recommend you call your insurance and ask if they cover our services for inpatient appointments. Do not rely on internet based information.
No Insurance:
If you do not have insurance or your insurance is not accepted at FMNW, then payment is due at the time of service. Time of service payments may receive a 20% discount off the doctor's rates.

CANCELLATION POLICY:
FMNW has a 48-hour cancellation policy. Failure to cancel your appointment 48-hours or more in advance will result in a $50 no show fee. This fee will not be billed to insurance and it will be the patients responsibility to pay the billed amount.
